If you’re a GP, expect timely letters about your patient’s improvements. If you’re a support coordinator or case manager, we’re happy to discuss goals and ensure our service complements other supports.

We understand the ins and outs of NDIS, Medicare plans, and aged care funding. We’ll make sure to provide the documentation your client needs for plan reviews or auditing. Our office can assist with service agreements and reporting requirements seamlessly.

Collaborative Approach

We view referrals as the start of a partnership. Our Exercise Physiologist will collaborate with you and other members of the client’s care team. This might involve:

  • Joining case conferences or team meetings if needed (we’re happy to provide input on exercise goals for care plans).

  • Aligning our exercise program with any existing therapy goals (physio, OT, etc.) to ensure consistency and avoid duplication.

  • Referring back or to other services if we identify needs outside our scope – for example, if a client would benefit from a dietitian or psychologist, we’ll communicate that to the GP or coordinator.

  • Being accessible for questions – if you’re ever unsure about our approach or want an update, just reach out. We are approachable and responsive.
